At the far end of the hall is a door you open to go to the war table. Pull up the quest map. Press the Triangle button. Look for the icon for Haven. Before you can do anything else you will have to go speak with Cassandra in the War Room. After they get done talking you will have a quest to meet up with a few of the Inquisition key members including the Blacksmith.

Go to his area outside of the main city and talk to him. After you are done talking to him you should see a golden standard in the smith that you can buy and sell items at. The only difference is that one is basically a free respec and the other is infinite but costs a bit of gold.

After you choose one go to the accessories screen on the character you want to respec and equip the amulet. The amulet breaks after you use it and you skills will be reset.
